I'd say it's a intake lobe worn off,and the exhaust valve is being sucked open.

I'm not so sure thats inside the engine.

I would be looking closely at the water pump and harmonic dampner for loose pullys and checking the flexplate for loose fastners. Also, the exhaust system and its hangers... might be hitting on something.

I think this might be a good time to get out the broom handle and use it as a stethescope.

If all else fails, a compression test and a leak down test will tell you pretty quickly if your guess it correct.

I thought the noise got louder the closer the guy with the camera got to the driver side tail pipe.

Take off the valve covers & check rockers and pushrods, go from there.
